How to Apply

All prospective applicants must be nursing facilities, assisted living facilities, intermediate care facilities, or similar in nature located in New Mexico.

Applicants need to provide the following information in their application:

  • Three years of loss runs

  • Payroll audit

  • Indemnity agreement

  • Payroll questionnaire
